JOB SUMMARY:
| Identification of barriers while serving as a liaison in building
cultural understanding for community, provider and client.

ESSENTIAL
FUNCTIONS:
1. Cultural
mediation among individuals, communities, and health and social service systems
A.
Establish better communication processes
B.
Enhance care quality by aiding communication between provider and patient to clarify cultural practices
2.
Providing culturally appropriate health education, information and effective communication
3.
Care coordination, case management, and system navigation
A.
Assist in coordination of health assessments with Family Planning team or other community providers
B. Make referrals and coordinate services
C. Facilitate continuity of care by providing follow up
D. Link clients to and inform them of available community
resources
4. Advocating for individuals and communities
A. Empower participants in self and community advocacy
B. Articulate and advocate needs of community and individuals
to others
5. Building individual and community capacity
6.
Providing direct service
A. Promote wellness
by providing culturally appropriate health information to clients and providers
B.
Educate clients on disease prevention as directed within Scope of Practice
C.
Conduct health related screenings (i.e. heights, weights, diabetes, hypertension, dental varnishing)
D. Organize and /or facilitate groups to meet participants
where they are
7. Conducting outreach and
referrals according to CHW Scope of Practice
8. Participating
in evaluation and needs of individual and/or community
NON-ESSENTIAL FUNCTIONS:
Perform other duties as assigned. May require some flexible hours
outside regular office hours.
EDUCATION:
High school diploma or GED-Current CHW Certificate or able to obtain within 18 months of hire.
SKILLS / ABILITIES:
Bilingual in English and Spanish preferred
other languages welcome. Ability to maintain confidentiality, communicate both orally and written. Good time management
and organizational skills. Valid Minnesota Driver’s License. Available for travel as job necessitates. Empathy for people
of various walks of life and ability to be resourceful related to Social Determinants of Health. Able to lift and carry supplies
for community events and clinics. Standing and walking, able to multi-task and follow through and comfortable in non-traditional
settings.
